We are located at the Forum in Delano, MN at 125 Woodland Road, Suite #150. Our clinic is open Monday, Wednesday, and Friday from 8:45–11:15 AM and the option for thoughtfully arranged accommodations when your care requires added flexibility.

We’ve partnered with Any Lab Test Now in Rogers and Plymouth, MN to offer our members exclusive, reduced-cost lab services. This partnership makes it simple and affordable to stay on top of your health with member-only pricing on all lab testing.

Imaging discounts are available throughout the metro with RAYUS Radiology. They offer numerous locations and heavily discounted imaging for patients.
New Services Ahead
We’re continuously growing to better serve our patients. Within the next year, we plan to expand our services to include IV hydration, vaccine clinics, and even more in-office procedures such as toenail removal and basic dermatology care. Our goal is to provide more convenient, comprehensive, and affordable healthcare — all under one roof.
