Clinic Hours and Location
We are located at the Forum in Delano, MN at 125 Woodland Road, Suite #150. Our clinic is open Monday, Wednesday, and Friday from 8:45–11:15 AM with additional coordination available whenever your care needs extend beyond standard hours.
Discounted Labs and Imaging
We’ve partnered with LabCorp as well as Any Lab Test Now in Rogers and Plymouth, MN to offer our members exclusive, reduced-cost lab services. This partnership makes it simple and affordable to stay on top of your health with member-only pricing on all lab testing.

Imaging discounts are available throughout the metro with iCare Diagnostics and RAYUS Radiology. They offer numerous locations and heavily discounted imaging for patients.
New Services Ahead
We’re continuously growing to better serve our patients. Within the next year, we plan to expand our coverage with a physician while having a limited patient panel to ensure our patients get the access they deserve. Our goal is to provide more convenient, comprehensive, and affordable healthcare — all under one roof.
